Stable housing for all makes for a stable community.
You may qualify if:
You are enrolled in NJ Family Care (Aetna, Fidelis, Horizon, United, Wellpoint)
Have medical or behavioral health needs
Are experiencing homelessness, housing instability, or are at risk of losing housing
*Referrals are welcome from healthcare providers, social service agencies and community partners.
